This is a great little form I use on my clans website for recruitment. The form once data has being put in and submited is sent to the email address of your liking ( example: feedback@yourdomain.com ). You can edit the input fields to your liking and feel free to edit how you like. The form is used on my gaming clans site for recruitment and it does a great job for recruits making first contact giving us a basic background of them before we talk more.

Installation: Just edit the feilds ie email info and form layout using dreamweaver, gedit or even notepad. The script explains itself.
